Be sure to read the information at the top of the page on the form.
If you have a picture of the veteran in uniform, please include a copy of the picture.
NOTE:
If you bring the documents to the library during hours we are open, we have the capability to scan the picture and the veteran’s discharge paper(s) if needed.
